Cover is offered on a 12-month basis, with the starting date determined the participant.

Once you fill and sign the form, please send it as an email attachment to unasi254ag@gmail.com. for signature and rubber-stamping, and forwarding to Old Mutual further action. As for the queries that you may have in the meantime regarding the premium payable on the basis of your age, family size and the benefits that you and your family wish to pay for, please contact Prof. Richard Simwa, the Principal Officer (PO) of Unasi Insurance Agency Ltd, via WhatsApp -- through this number: 254-722771902. Following the determination of the premium amount due from you, you will make all your payments directly to a bank account specified by Old Mutual.
